How your personal information is collected

Personal information is provided directly by you when you make contact with me by telephone, SMS, email or via my website or FDG website. By contacting me, you give consent for me to collect this personal data in order for me to provide a Nutrition and Dietetic Service to you.

I may also collect other personal information from a number of different sources such as: other clinicians and healthcare professionals, General Practitioners (GPs), hospitals (including NHS and independent providers) and independent health and lifestyle centres. This may include clinical information about your past and present health, diagnosis and medication.

What I do with your personal information

As a healthcare provider, I am subject to a range of legal and regulatory responsibilities. I am also under a legal and ethical obligation to maintain clinical records safely and securely. Your personal information is required for professional clinical record keeping. I may also need to share your personal details and information about your treatment with other healthcare professionals in order for them to provide safe and effective care.

In some circumstances, your personal information may also be processed for the purposes of local and national clinical audit, which may be carried out in order to assess outcomes of treatment and what improvements could be made for the future. You are, however, entitled to object to the use of your personal data for this purpose.

Your personal information will not be distributed to third parties unless I have your consent or I am required by law to do so.

Your personal information will be kept for 8 years after your last consultation unless you ask me to remove it; after this time, your personal information will be destroyed.

Access to your personal information

You have the right to request a copy of your personal information and to ask me to rectify any personal information that you think is inaccurate or incomplete.

How you can withdraw consent

You have the right to withdraw consent if you do not wish for your personal information to be used in this way. Please contact the data controller listed below.

How I will communicate with you

I may get in touch with you in several ways including telephone, SMS, email (unencrypted) and/or post. If I contact you using the telephone number(s) you have provided (landline or mobile) and you aren’t available, then I may leave a voice message or SMS text. This will only contain sufficient basic details so that you can identify who the call is from and how to call me back.
